Boarding Registration

I understand that there is an additional charge of $3.00 per pet per day if my pet is eating the kennel’s food. I also understand that if my pets are eating their own food and the supply runs out, I will be charged an additional $3.00 per pet per day to use the kennel food.

I understand that if my pet needs medications administered while boarding there is an additional charge of $6 per pet per day.

I understand that drop-offs or pick-ups outside of business hours must be scheduled ahead of time. These are to only take place at 8:30am or 5:30pm ONLY. There is an additional charge of $28.00 per pet. If applicable, during holidays there is an additional charge of $8.00 per pet.

If I am boarding more than one pet, I would like them boarded in the same run and give Plateau Animal Hospital permission to do so.

I understand that if my pet needs medical or emergency care, Plateau Animal Hospital will make every attempt to reach me or my emergency contact. In the event that we are unable to reach you or your contact we will proceed with medical treatment for your pet. I authorize the emergency contact listed above to act as my representative if medical or emergency decisions need to be made on my behalf.  I understand and accept that I will be responsible for all fees incurred.

I will not hold Plateau Animal Hospital responsible for any personal items lost or damaged while boarding.
